Goal: Help people earn steady incomes that allow them to meet their health needs.

Education Access and Quality

The opportunity to attain a quality education plays a significant role in health outcomes.

Goal: Increase educational opportunities and help children and adolescents do well in school.

Health Care Access and Quality

Having access to preventive and treatment services is crucial for maintaining health.

Goal: Increase access to comprehensive, high-quality health care services.

Neighborhood and Built Environment

Safe neighborhoods with access to parks, healthy food options, and clean air are essential for well-being.

Goal: Create neighborhoods and environments that promote health and safety.

Social and Community Context

Social support networks, feelings of safety and belonging, and exposure to violence all influence health.

Goal: Increase social and community support.

What Makes Us Unique?

We approach the SDOH challenges from new perspectives to come up with outside-the-box solutions - in many cases, one-of-a-kind software/hardware solutions, leveraging latest technologies - and provide them at no cost to our Community-Based Organization (CBO) partners in major cities, so that they can utilize them in their community engagements related to the SDOH. This way, we are able to accelerate the national impact of our efforts and bring mutual benefits to us and our CBO partners.

Our efforts are focused on improving health outcomes for a wide range of communities, each facing unique challenges. We are committed to serving:

VETERANS: We recognize the unique healthcare needs of veterans, including mental health support, access to specialized care, and social reintegration.

LOW-INCOME FAMILIES: Economic instability often limits access to quality healthcare, housing, and education.

RACIAL AND ETHNIC MINORITIES: Health disparities among racial and ethnic minorities remain a significant challenge.

PEOPLE WITH DISABILITIES: Individuals with physical or mental disabilities often face barriers to healthcare and social participation.

ELDERLY POPULATIONS: As people age, they encounter increased healthcare needs and the risk of social isolation.

HOMELESS INDIVIDUALS: Homelessness is both a cause and consequence of poor health.
